Richard Podlesak Memorial Scholarship Application

Please consider this as my application for a 2019 fall semester scholarship sponsored by the Heartland Chapter of the Association of Certified Fraud Examiners. I understand the scholarship(s) will be awarded on the basis of overall academic achievement, letters of recommendation, and a brief essay on why I desire to receive the scholarship and how fraud awareness will enhance my professional career. I am a full-time undergraduate (at least 12 hours per semester, or equivalent on a quarterly or otherwise basis) or graduate student (at least 3 hours per semester) at an accredited college or university (community colleges included) majoring in accounting, business, criminal justice or similar field of study as determined by the scholarship committee, and have at least a 3.0 or better grade point average


Applications must be submitted by June 30, 2019.

I agree with the requirement for entering into the “Service Agreement” with the Chapter and all other requirements associated with the receipt of this scholarship.

I certify that information contained within this application and my enclosures is true and accurate.

I understand that I must contact the Heartland Chapter ACFE to receive the name of  Chapter member, or members. It is my responsibility to meet with a Chapter member or attend a Chapter meeting for a possible recommendation.

Board interviews will be held in August 2019, and the Scholarship award is slated to be announced at the Heartland Chapter ACFE meeting September 10, 2019.
